To request a sick note for work
Fit notes are sometimes referred to as medical statements, a sick note or a doctor’s note.
- 7 days off sick or less – if you’re off work sick for 7 days or less, your employer should not ask for medical evidence that you’ve been ill. Instead, they can ask you to confirm that you’ve been ill. You can do this by filling in a form yourself when you return to work. This is called self-certification.
- More than 7 days off sick – if you’re off work sick for more than 7 days, your employer will usually ask for a fit note from a healthcare professional. If you need a fit note, contact the healthcare professional treating you. This should be a doctor, nurse, occupational therapist, pharmacist or physiotherapist.

Register for patient access online services
Patient access online services provides patients with access to several services including:
- book, change or cancel appointments
- view test results
- contact us for advice and support
- order repeat prescriptions
- view your health record, including information about medicines and vaccinations
- see communications between our surgery and other services, such as hospitals
To use online health services, you need to register and prove your identity. You’ll need to:
- Prove who you are (you may be able to do this over the phone):

- provide photo identification and proof of address (for example, a passport or driving licence and a bank statement or council tax statement), or
- if you have no ID but are well known to the surgery, a member of staff may be able to confirm your identity, or
- if you have no ID and are not well known to the surgery, the ability to answer questions about the information in your GP record may confirm the record is yours.

Friends & Family Test (FFT)
The NHS Friends and Family Test (FFT) was created to help service providers and commissioners understand whether patients are happy with the service provided, or where improvements are needed. It’s a quick and anonymous way to give your views after receiving NHS care or treatment. Feedback can be provided by completing the form below or by requesting a hard copy from reception.
Surgery policies
Our surgery follows different policies (a set of ideas or plans) that are used as a basis for making decisions. Our policies are:
- Chaperones – all patients are entitled to have a chaperone present for any consultation, examination or procedure where they consider one is required. The chaperone may be a family member or friend, but on occasions a formal chaperone may be preferred.
- Accessible Information Standard. The Standard sets out a specific, consistent approach to identifying, recording, flagging, sharing and meeting the information and communication support needs of patients, service users, carers and parents with a disability, impairment or sensory loss.
- Suggestions, comments and complaints – to make a complaint or comment on the provision of our services.
- Zero Tolerance – for action that may be taken where a patient is violent or abusive.
- Patient information – how your information will be used and your rights.
Care Quality Commission (CQC)
The role of the CQC as an independent regulator is to register health and adult social care service providers in England and to check, through inspection and ongoing monitoring, that standards are being met. All GP practices in England must be registered with the CQC.
Surgery Profiles
Surgery profiles are designed to support surgeries, primary care networks (PCNs), integrated care services (ICSs) and local authorities to ensure that they are providing and commissioning effective and appropriate healthcare services for their local population.
Surgery Quality & Outcome Framework
The Quality and Outcomes Framework (QOF) is a voluntary annual reward and incentive programme for all GP practices in England, detailing practice achievement results. It is not about performance management but resourcing and rewarding good practice.
